A 45-year old male was clinically admitted for for work up for bilateral pleural effusion. He was noted to have fever after admission. Pleural tapping c/st: no growth, cell count not done due to blood stained specimen, blood c/st: no growth. ICU was consulted for increased O2 requirement and hypotension. His past medical history was related to the CXR finding. What was it?
