Case History: A 29- year-old unemployed lady was admitted through the chest clinic to Ruttonjee Hospital in Februrary 2006 for cough for few months. She was a non-smoker, non-drinker, with past history of paranoid schizophrenia, and was on flupenthixol 40mg IM monthly, diazepam 2mg nocte, benzhexol 2mg BD, valproate 400mg nocte. Her cough was associated with whitish sputum production and constitutional upset. She has no fever or hemoptysis. Physical examination was unremarkable except minimal basal crepitations.
