Dear All CCM trainees,
Please be informed that our 21st CCM Exit and Annual Assessment will be held in the afternoon of 14th November 2009. If anyone of you would like to sit in the Assessment, you have to submit application form duly completed (Forms attached) on or before 14 September 2009 to me via return email or by post at Rm.75, 2/F, Main Block, PYNEH, 3 Lok Man Road, Chai Wan, HK.
Please note that late application will NOT be entertained.
