M/ 50 was admitted in septic shock. CXR showed right opacity. USG right chest was done and showed this. What is this and what would you do next?

Answer: The above USG chest image showed a large pleural effusion. However there are echogenic debris floating around in the effusion, indicating likely a complicated parapneumonic effusion or emphyema. Chest Drain was inserted and purulent effusion was obtained. Patient recovered well afterwards.
