I am writing to inform you that your HKSCCM membership will expire on 31 December 2009. For next term of membership, HKSCCM council decided to extend it into a two-year basis, therefore from 1st January 2010 to 31st December 2011 inclusively.
Kindly forward the completed membership form PLUS cheque payable to HONG KONG SOCIETY OF CRITICAL CARE MEDICINE LIMITED to the correspondence as charted in the attached letter/ application form.
