M/39 Abdominal distension after renal transplant

A 39 year-old man suffered from ESRF due to IgA nephropathy. Cadaveric renal tranplant was performed.

Five months after renal transplantation, he started to complain of progressive abdominal distension and fever. Ascites fluid was grew MRSA, and a course of Vancomycin was given for 2 weeks. He then developed few episodes of small bowel intestinal obstruction, which subsided with conservative treatment.

Two months later, he was admitted into the renal ward, complaining of increasing abdominal distension again and very poor appetite. He was septic clinically, and creatinine rose from 150 to 350 umol/L, with HCO3 11 mmol/L and pH 7.20. ICU was consulted to consider offering CRRT to the patient. A clinical diagnosis of sclerosing peritonitis was suspected, and therefore CT abdomen was done. Air-borne precaution was taken because of numerous nodules on CXR.

Some striking features had shown up on the plain CT….
