Quoted from the US FDA: "The U.S. Food and Drug Administration (FDA) is reminding healthcare professionals of an increased mortality risk associated with the use of the intravenous antibacterial Tygacil (tigecycline) compared to that of other drugs used to treat a variety of serious infections. The increased risk was determined using a pooled analysis of clinical trials. The cause of the excess death in these trials is often uncertain, but it is likely that most deaths in patients with these severe infections were related to progression of the infection."

Purpose of review: Invasive candidiasis remains an important infection for ICU patients, associated with poor clinical outcomes. It has been increasingly recognized that the traditional paradigm of culture-directed antifungal treatment is unsatisfactory, and that earlier antifungal intervention strategies, such as prophylaxis, preemptive therapy, and empiric therapy, are required to improve patient outcomes. The purpose of this review is to summarize the recent supportive evidence for such strategies and to highlight the current challenges in their implementation.

Aspirin and clopidogrel are mainstays of therapy for patients presenting with an acute coronary syndrome. National guidelines dictate that at the time of the patient's presentation to an emergency department, these therapies should be given expeditiously, whether or not percutaneous coronary intervention is planned.

In this issue of the Journal, Mehta et al. report on the results of the Clopidogrel and Aspirin Optimal Dose Usage to Reduce Recurrent Events–Seventh Organization to Assess Strategies in Ischemic Syndromes (CURRENT–OASIS 7) trial, which evaluated alternative dosing regimens for both of these agents.1
